Delete Your My Simple Report Account

Steps to delete your account

1. Send an email to support@mysimplereport.com with the subject line Delete My MySimpleReport Account.
2. Include the email address used for your MySimpleReport account if you are contacting us from a different email address.
3. We may ask you to verify account ownership before deletion is completed.
4. After verification, we will delete your account and eligible associated data from active systems within 30 days.

Data that is deleted

When your deletion request is completed, we delete or de-identify data associated with your MySimpleReport account, including:

• Your account profile information, such as name, email address, phone number, and account preferences.
• Uploaded medical reports, documents, images, files, and generated summaries.
• Patient profile information, family member records, medications, insurance details, vault documents, and connected records stored in your account.
• Report sharing links, QR profile data, and other user-created health record content.

Data that may be kept

Some limited data may be kept when required for security, legal, tax, billing, fraud prevention, dispute resolution, or compliance purposes:

• Payment, invoice, and billing records may be retained for the period required by applicable law.
• Security, audit, and system logs may be retained for up to 90 days.
• Backup copies may remain in encrypted backups for up to 90 days before being overwritten or deleted.
• De-identified or aggregated analytics that cannot reasonably identify you may be retained to improve service reliability and safety.
